Initial Evaluation (60 minutes)                      $160                               

Individual Psychotherapy (50 minutes)       $140       

Insurance

I am a provider for the following insurances:

•          Blue Cross Blue Shield

•          Harvard Pilgrim

•          Tufts

•          United Healthcare

You are responsible for co-pays and deductibles. Contact your insurance company to familiarize yourself with your benefits. Consider asking the following questions:

What are my mental health benefits?

How many sessions per year does my insurance cover?

Do I have a deductible and has it been met?

Do I have a co-pay?

Do I need a referral from my primary care physician?

Payment

I accept checks and credit cards. Payment is required at the time of service.

Out-of-Network

Many insurance companies will reimburse you for out-of-network providers. Contact your insurance company to see if your plan offers this coverage. I would be happy to provide you with the necessary paperwork to submit to your insurance company.

Cancellation/No Show Policy

I require that you notify me of any cancellations 24 hours in advance and on the prior business day before your scheduled appointment. If you miss your appointment or do not provide me with such notice, you will incur a $75 fee since I am unable to bill the insurance company for that lost hour.
